Health systems interested in improving care for older adults at their organization can learn how you can implement the 4Ms Framework for Age-Friendly care, an evidence-based model to support the needs of the aging community in a clinical setting. The 4Msare: What Matters, Medications, Mentation, and Mobility.
Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States(CHA).
