ABOUT THE PROJECT
The HealthMatters Program Scale-Up research project will examine the challenges and opportunities to successful state-wide roll-out of the HealthMatters Program in community based organizations (CBOs) in the states of Alaska, Illinois, Kentucky and Missouri. The research project has the following goals:

- Test the success of a scale-up plan across three states to determine processes that facilitate maximal reach and impact of the HealthMatters Program.
- Understand the capacity needed to support local CBOs in implementing the HealthMatters Program within each state.
- Evaluate the impact of the HealthMatters Program for people with I/DD and their support persons.
