The HealthMatters, Illinois! is a health promotion initiative aimed at improving health and health promotion of people with intellectual and developmental disabilities (I/DD) throughout the state of Illinois. HealthMatters Program team has partnered with Illinois Disability and Health Program at the Illinois Department of Public Health to assist provider organizations with promoting healthy lifestyles to people with I/DD while meeting the requirements of the National Core Indicators.

WHO CAN PARTICIPATE?
We are looking for motivated Organizations who provide Supports for Community Living services to people with I/DD in Illinois to participate in this initiative.
Up to 15 provider organizations will be selected to participate and will receive resources to benefit people with I/DD receiving services, the organization, and their employees. Each organization will need to devote the staff to make up a team that includes but is not limited to: two (or more) staff leaders and a HealthMatters Team Coordinator who will ultimately become Certified Instructors to teach 12-week HealthMatters Program to people with I/DD.

WHAT WILL YOU DO?
Organization needs to provide structured time for a 12-week HealthMatters Program for people with I/DD, staff, and Wellness Committee to meet. This project is one (1) year long and requires the following commitments:

Wave 3: February 2018 - February 2019

ACTIVITY DEADLINES
Create a Wellness Committee February/March 2018
Complete organizational HealthMatters Assessments (staff only, at least 25% of workforce, must be completed at least 2 weeks before webinars). March 16th, 2018 (Time 1)
March 16th, 2019 (Time 2)
Create and implement Strategic Plan for health promotion. April 2018-February 2019
Attend three (3) 90-minute HealthMatters Train-the-Trainer Webinars and complete 90 minutes of offline work between webinars (minimum 3 participants). Option 1: April  9, 11, 13 (10am-11:30am CST)
Option 2: April 9, 11, 13 (1pm - 3 :30pm  CST)
Option 3: April 16, 18, 20 (10am-11:30am CST)
Option 4: April 16, 18, 20 (1pm - 3 :30pm  CST)
Run at least one 12-week HealthMatters Program (3 times per week, 4-6 hour per week, minimum 6 participants with I/DD) Start within 2 weeks of the last Webinar.
